When: As early as desired (pre-planning, diagnosis, or period before decline).
Outcome: Clear advance plan, affairs in order, practical supports in place, and meaningful legacy beginnings.
A – Accompany & Assist
Provide steady, non‑medical presence and assistance. Adapt plans as needs evolve, and connect with clinical and community supports.
Includes: Regular visits/check‑ins; companionship and caregiver coaching; anticipatory‑grief education; facilitating family communication and relationship repair; assisting on legacy projects; coordinating with hospice/palliative teams; maintaining scheduling and a shared Care Journal; emotional, spiritual, and practical support.
When: During progressive decline and episodic needs.
Outcome: One coordinated care plan, calmer household, informed family.
R – Reassure & Steady
Hold respectful, person‑centered space at the bedside to facilitate comfort, meaningful rituals, and farewells during transition. Provide clear, practical guidance and immediate next steps at the time of death.
Includes: Bedside companionship and advocacy; comfort practices (breathwork, music, guided presence); supporting rituals and farewells; caregiver respite; assisting with notifications, funeral/mortuary coordination, and honouring body‑care wishes.
When: Hours to days of active dying and vigil.
Outcome: A dignified, person‑centered transition and practical clarity for family.
E – Embrace & Remember
Aftercare to support emotional adjustment and complete practical and legacy‑related tasks after the transition.
Includes: Help with memorial planning; completing or handing off legacy projects; scheduled follow‑up check‑ins; normalizing grief responses; referrals to bereavement counselling, peer support, and community resources as necessary.
When: Days to months after death, as needed.
Outcome: Continued emotional support, completed legacy work, and resources for grief.
