Surface and document values and wishes, begin legacy projects, and co‑create an Advance Care Plan with practical supports in place.

Includes: Intake conversation; documenting wishes, emergency contacts, advance care plan; initiating legacy projects; arranging household supports and community resources.

When: As early as desired (pre-planning, diagnosis, or period before decline).  

Outcome: Clear advance plan, affairs in order, practical supports in place, and meaningful legacy beginnings.
