Includes: Regular visits/check‑ins; companionship and caregiver coaching; anticipatory‑grief education; facilitating family communication and relationship repair; assisting on legacy projects; coordinating with hospice/palliative teams; maintaining scheduling and a shared Care Journal; emotional, spiritual, and practical support.
When: During progressive decline and episodic needs.
Outcome: One coordinated care plan, calmer household, informed family.
